An anti-inflammatory cuts the signal that produces pain. Resolution is something else: it is the biological programme that closes the inflammatory process in an orderly way and leaves the tissue repaired. Blocking brings relief sooner; resolving finishes the job. And if you train, that difference shapes your adaptations.

For decades we treated inflammation as an enemy to be defeated: if it hurts or swells, switch it off as fast as possible with anti-inflammatories. But the science of the last twenty years has revealed something rather different and a good deal more interesting: inflammation does not end because it passively fades out, but because the body activates a programme of its own to resolve it. And that programme has protagonists with a name: SPM, or Specialised Pro-resolving Mediators.

Inflammation is not the problem: it is the response

When you train hard, pick up an injury or your body fights an infection, inflammation is the first line of defence. It is a necessary process: it increases blood flow, recruits immune cells and prepares the tissue to repair itself. The problem is not that inflammation appears, but that it stays longer than it needs to.

For a long time it was assumed inflammation resolved simply because the signals that started it diluted over time, passively. The work of Charles Serhan at Harvard University changed that paradigm in the early 2000s: he discovered that resolution is an active process, orchestrated by specific molecules the body makes to signal mission accomplished and set the clean-up and repair of tissue in motion.

Blocking versus resolving: a difference with consequences

What a conventional anti-inflammatory does

NSAIDs work by blocking enzymes, mainly COX, that produce pro-inflammatory molecules. It is an effective strategy for short-term symptom relief, but it has an important limitation: they do not accelerate resolution. And there is published work suggesting their systematic use may interfere with the natural signals that set it in motion (Peake et al., 2017; Markworth et al., 2020).

What SPM do

SPM work the other way round. Rather than suppressing the inflammatory response, they take part in closing it in an orderly way: slowing the arrival of new inflammatory cells, encouraging the exit of those that have done their job, and opening the door to the repair phase.

They also do something no anti-inflammatory does: they activate a clean-up programme called efferocytosis, through which macrophages clear away cellular debris and immune cells that are no longer needed, leaving the tissue ready to regenerate.

It is the difference between silencing an alarm and switching off the system that set it off so the building can go back to normal.

Why this matters if you train

High-intensity training generates muscle micro-damage and a controlled inflammatory response as part of the normal adaptation process. That process is desirable: it is partly how a muscle gets stronger. The aim is not to eliminate inflammation, but to make sure the body has what it needs to close it at the right moment.

When that closure happens on time, the tissue moves into the repair phase sooner and the body is available for the next stimulus. When it does not, the response drags on and the adaptation cycle suffers. It is the difference between stringing sessions together and dragging yourself through them.

And there is one detail that governs all of the above: SPM are synthesised from EPA and DHA, but that conversion is not always efficient. If the body does not complete it properly, it can fall short of these mediators precisely when recovery demand is highest. Frequently asked questions

Is it bad to take ibuprofen after training?

Occasionally, for a specific complaint, it is not an issue. The nuance is in making it routine: NSAIDs relieve pain but do not accelerate recovery, and several published papers suggest chronic use after exercise may interfere with training adaptations, because inflammation is part of the signal that triggers them. Frequent or prolonged use of these medicines is also associated with a higher risk of adverse effects.

Are SPM anti-inflammatories?

No, and the distinction matters. An anti-inflammatory blocks the production of pro-inflammatory molecules. SPM block nothing: they take part in the phase that closes the process once it has done its job. One interrupts, the other finishes.

What is efferocytosis?

The process by which macrophages clear away cellular debris and immune cells that have finished their work in the inflamed area. It is the step that leaves the tissue clean and ready to regenerate, and SPM are one of the signals that activate it.

Does blocking inflammation limit strength gains?

The available research points that way when the blocking is systematic and prolonged, not from an isolated dose. The inflammatory response to training is part of the mechanism by which muscle adapts, so suppressing it continuously may take away part of that stimulus.

What is the difference between good and bad inflammation?

Not all inflammation is harmful. Acute inflammation is an essential part of the natural healing process: it clears damaged cells, deals with potential threats and starts tissue repair. The problem arises when it does not resolve properly and lingers at a low level for longer than needed, which can prolong recovery and contribute to chronic tissue stress. SPM do not block or suppress inflammation: they actively support the body's natural resolution phase.
